Palliative gr advocates that palliative care must be available and accessible in a timely, comprehensive and uncomplicated manner for all people in an advanced palliative care situation and their relatives in the canton of Graubünden. Whenever possible, this should take place at the place of choice of the person concerned. The extent and intensity of care and accompaniment can vary individually and correspond to the needs of the person concerned.

The Palliative Care Unit is part of the Internal Medicine Department of the Cantonal Hospital Graubünden. On the ward at the Fontana site, patients with an incurable disease or chronic pain receive interdisciplinary and comprehensive care. The stay in the palliative ward is intended to support patients with a complex situation (pain symptoms, decision-making in a palliative situation) and to enable a return to the patient's preferred environment. In addition to the ward, there is also an outpatient consultation hour, where patients with the focus on pain, nutritional problems and the like can be cared for.

Palliative Bridge Service

Since January 1, 2013, this outpatient service has been offered as a pilot project in the canton of Graubünden. The employees of the palliative bridge service accompany and advise patients, their relatives and the persons and organizations involved in care and support in complex questions regarding care and support. The professional support can be requested by the patient, the relatives, the family doctor or the Spitex services and nursing homes. The supplementary offer by the palliative bridge service is provided temporarily until the caregivers involved are able to cope with the situation themselves.

**Hospice Grisons

In the canton of Graubünden, terminally ill and dying people can be cared for and nursed in the hospice in Maienfeld. The facility has four beds and is affiliated with the Senesca care center in Maienfeld.

The Hospice Graubünden closes a gap in the palliative care network, especially for younger seriously ill people. Affected persons and relatives find competent support during a respite stay or in the last phase of life.

The Voluntary Accompaniment of the Seriously Ill and Dying is available to seriously ill people and supports and relieves their next of kin with the aim of caring for the affected patients at home until their death. This according to the motto: "Live well until the end and die in peace." This important and demanding work is done exclusively by volunteers. The free care can be provided for hours or night watches.
